JCC Athletic Passes
JCC Athletic Passes are available for purchase at the High School or Middle School offices.
Adults - $75.00
Students - $40.00
These passes give you admittance to all home JCC Athletic Events for the 2015 - 2016 school year. (excluding playoffs & section events).
All JCC athletes must have the following completed document on file in the Athletic office to be eligible to participate in all sports. A copy of these documents can be downloaded and printed from the links at bottom of this web page.
2015-2016 Physical Exam Form (these are good for 3 years)
2015-2016 MSHSL Athletic Eligibility Form.
2015-2016 Consent for Treatment Form
2015-2016 Insurance Lunch Waiver Form (to qualify for reduced athletic rate, you MUST fill out and qualify with the 2015-2016 lunch application form.
Activity Fees Are As Follows.....Please pay for each sport separately. FAMILY MAXIMUM $350.00
HS Level Football $90.00 - F/R $25.00
Other HS Level Sports $80.00 - F/R $25.00
MS Level Sports $55.00 - F/R $15.00
Other Participation (7-12) $45.00 - F/R $15.00 - including Fall Play, 1-Act Play, Spring Musical, Swing Choir, Jazz Band, Speech, FFA, Math League, Knowledge Bowl, Mock Trial, Robotics, etc.
